CPCS Re-Activation

Reinstatement of the CPCS credential:

If your CPCS credential has become inactive, you may apply for reinstatement by contacting [email protected] for personal assistance.

1.Reinstatement (Re-Activation) application  CLICK HERE
2.Reinstatement fee 
3.Any past-due fee(s)
4.Must know your CPCS number
5. NEW  12 hours of CE training in Clinical Supervision which includes at least six (6) hours of Ethics. May not reuse CEs from original application or re-certification 

6.  Take and pass the CPCS Recertification Exam (the link to the exam is on the Re-Activation Form)
